Why does my hip hurt? Why does my hip hurt when I walk? Why does my hip hurt when I lie on my side?

Published Updated

Hip pain is a broad description, not a diagnosis. The pain may feel deep at the front of the hip or in the groin, sit over the outer side of the hip, spread into the thigh, or be felt in the buttock. A change in walking, climbing stairs, turning in bed or lying on one side can offer useful clues, but it cannot identify the cause by itself. Start with a short record of where the pain is strongest, when it began, what you were doing at the time, and whether sleep, walking or putting weight through the leg has changed. That gives a clinician a much clearer starting point than a pain score alone.

Preferred clinic

Appointments are confirmed through WhatsApp. If you have language requirements, please mention them in your enquiry.

The short answer

Hip pain may involve the joint, tendons, muscles or bursae around it, or pain referred from the lower back. Pain with walking, stairs or side-lying helps describe the pattern but does not confirm the cause. Record the location, trigger, stiffness and effect on daily function. Seek prompt medical assessment for severe pain after injury, inability to walk or bear weight, numbness after injury, or a hot, swollen hip with fever or feeling unwell. [1]

Why does my hip hurt? Why does my hip hurt when I walk? Why does my hip hurt when I lie on my side? Start with pain location

The word "hip" is often used for a large area. Before deciding what it means, use one finger to mark the most painful place: deep in the front crease, in the groin, over the prominent outer bone, in the buttock, or down the upper thigh. Also describe whether it is an ache, sharp catch, burning feeling, stiffness or a sense of weakness. These details do not diagnose arthritis, a tendon problem or a spinal problem, but they help prevent every pain around the pelvis from being treated as the same complaint. For a broader overview of possible patterns, see hip pain causes.

Location should be read alongside the timeline. Sudden pain after a fall, awkward turn or impact has a different urgency from discomfort that has gradually appeared after a new running route, a period of lifting, or more time spent sitting. Mention a previous hip operation, osteoporosis, inflammatory illness, cancer treatment or medicines that may matter to the clinician. Do not use an internet pain map to rule out a serious problem. The hip is a weight-bearing joint, and several conditions can create overlapping sensations around it. [2]

Why does my hip hurt? Why does my hip hurt when I walk? Why does my hip hurt when I lie on my side? Front, groin and thigh clues

Pain felt at the front of the hip or in the groin can be associated with the joint itself, while symptoms may also run into the upper thigh. In older adults, front-of-hip, thigh and groin pain with stiffness can occur with hip osteoarthritis; front and groin symptoms may also occur with other conditions, including a labral problem. That is why a groin location is a clue rather than proof that the joint has "worn out". [3] If the question most concerns this combination, our page on hip pain and groin pain can help you organise the details to share at assessment.

Try to notice the task that exposes the symptom. Is it taking the first few steps after getting up, getting out of a low car, bending to put on socks, pivoting while carrying a bag, or taking a long stride? State whether the pain occurs on both sides or one side, whether it reaches the knee, and whether there is clicking, catching or a sense that the leg may give way. Those observations are useful for an examination, but should not be used to perform deep squats, forceful stretches or repeated twisting in an effort to reproduce pain.

Why does my hip hurt? Why does my hip hurt when I walk? Why does my hip hurt when I lie on my side? Outer hip and referred pain

Pain directly over the outer hip that is worse when you lie on that side may involve tissues around the hip, including bursae or tendons, but it still needs assessment in context. NHS guidance lists pain spreading to the upper thigh and worsening with movement or side-lying among patterns that can occur with bursitis. [1] The point is not to label every tender outer hip as bursitis. A clinician will also want to know whether the pain followed repeated climbing, a training change, a fall, or prolonged pressure while sleeping.

Buttock pain can be particularly confusing because the lower back can refer pain into the hip area and leg. A burning, electric or tingling sensation, altered feeling, or pain travelling below the knee should be described exactly, even if the back itself does not feel painful. HSS notes that pain in the back of the hip or buttock is often related to a spinal problem rather than the hip joint. [3] New weakness, loss of feeling or symptoms after an injury are not signs to simply monitor at home; they need timely medical advice.

Why does my hip hurt? Why does my hip hurt when I walk? Why does my hip hurt when I lie on my side? Use movement and function

A useful hip-pain record follows ordinary tasks rather than a single good or bad day. Compare flat walking, stairs, getting up from a chair, standing on one leg to dress, carrying shopping, getting in and out of a car, and rolling over in bed. Note whether the pain begins immediately, builds after activity, settles with rest, or returns at night. Function is important because it shows how the symptom affects your life, not only how intense it feels for a few minutes. The Hong Kong College of Orthopaedic Surgeons notes that walking, bending and getting in or out of a chair can become painful and difficult when hip mobility is affected. [2]

Keep the record short enough that you will actually use it. A dated note for several days can include the activity, the location, the approximate severity, what eased it, and any change in sleep or confidence on stairs. Add whether there was swelling, redness, bruising, a feverish feeling, numbness or weakness. This is not a test you must pass before seeking care. It is a way to make an appointment conversation more precise and to show whether the pattern is stable, worsening or becoming more limiting.

Why does my hip hurt? Why does my hip hurt when I walk? Why does my hip hurt when I lie on my side? Record walking and weight-bearing

When someone asks why walking hurts, distance and terrain matter. Record whether the pain appears on the first few steps, after ten minutes, only going uphill, while descending stairs, or after a long period of standing. Also say whether you have begun limping, shortening your stride, using a rail more often, or avoiding weight on one leg. A limp can protect a painful area in the short term but may also change how the back, knee and other hip are loaded. Rather than pushing through a clear increase in pain, reduce the provoking load and arrange an assessment if walking is persistently limited or worsening.

For mild symptoms without warning signs, general NHS advice is to keep moving if possible without overdoing it, avoid putting too much strain on the hip, and use an ice pack wrapped in a towel for up to 20 minutes every two to three hours. [1] That is general information, not a personal treatment plan. Avoid carrying heavy loads, and do not make a sudden return to long walks, hill training or intensive exercise simply because one day feels better. The focused guide to hip pain when walking offers additional questions to bring to a consultation.

Why does my hip hurt? Why does my hip hurt when I walk? Why does my hip hurt when I lie on my side? Note stiffness and daily impact

Stiffness has a useful story of its own. Write down whether it is strongest after sitting, when waking, after a journey, or after activity; include how long it lasts and whether gentle movement changes it. Explain practical effects: difficulty reaching feet for shoes, interrupted sleep, avoiding a favourite walk, taking longer on the MTR stairs, or needing help with a task that was previously routine. These examples give more useful information than saying the hip is "bad". They also help distinguish a manageable discomfort from a symptom that is steadily shrinking everyday life.

An assessment generally starts with your health history and a physical examination of movement, strength and alignment; imaging or other tests may be considered when clinically needed. [2] A Registered Chinese Medicine Practitioner may also ask about the pain pattern, sleep, digestion, prior health history and current medicines within a traditional assessment. That traditional approach is not the same as a biomedical diagnosis and should not replace evaluation for fracture, infection, neurological symptoms or another urgent cause. If acupuncture or Chinese medicine is considered, it should be discussed as an individual supportive option after the relevant safety and diagnostic questions have been addressed, not as a guaranteed fix.

Why does my hip hurt? Why does my hip hurt when I walk? Why does my hip hurt when I lie on my side? Know when to seek care

Hip pain deserves medical advice when it is getting worse, keeps returning, affects sleep or usual activities, or does not improve with sensible home measures. [1] Do not wait for a perfect description before asking for help. Say whether the symptom followed an accident, whether you can walk normally, and whether there is any heat, swelling, skin-colour change, feverish feeling, tingling or loss of sensation. These details help the service direct you appropriately. Pain that is becoming more frequent while walking or standing also needs attention rather than repeated self-testing.

The next step depends on the pattern. An acute injury or a concerning red flag calls for conventional urgent assessment. A persistent but non-urgent problem may need a clinician to examine the hip, back and leg and decide whether further tests, rehabilitation or another service is appropriate. A Chinese medicine consultation can be an option to discuss only after urgent concerns are addressed; be open about investigations, diagnoses, medicines and other care already underway. This approach supports coordination rather than asking one type of care to make assumptions about another.

Why does my hip hurt? Why does my hip hurt when I walk? Why does my hip hurt when I lie on my side? Urgent warning signs

Seek emergency medical help after a fall or injury if there is severe hip pain, you cannot walk or put weight on the leg, or you have tingling or loss of feeling in the hip or leg. [1] Severe sudden hip pain without a fall also needs urgent medical advice. A hot, swollen hip, a change in skin colour around it, or hip pain with a high temperature or feeling generally unwell can indicate a problem that should be assessed urgently. [1] Do not drive yourself if you need emergency care; use local emergency arrangements or ask someone to help you get there safely.

These warning signs are about safety, not about alarming every person with a sore hip. Most hip pain is not an emergency, yet serious causes cannot be excluded by how tough you are, how long you have managed it, or whether a home remedy briefly eased it. The useful question is whether the pattern includes trauma, inability to bear weight, rapidly increasing pain, altered sensation, systemic illness or obvious swelling and heat. If it does, prioritise medical assessment before massage, needling, vigorous exercise or attempts to manipulate the painful area.

Why does my hip hurt? Why does my hip hurt when I walk? Why does my hip hurt when I lie on my side? Discuss an appropriate next step

For a non-urgent enquiry with TCM Lab, prepare a brief summary: where the pain is, when it started, what walking or side-lying changes, any fall or change in exercise, current medicines, and care or scans already received. You can ask about an appointment at the Kwun Tong Chinese medicine clinic or the Chinese medicine clinic Prince Edward. Appointments are arranged by enquiry, so contact TCM Lab on WhatsApp at +852 6940 5663 to discuss availability and any language needs rather than assuming an immediate booking or a particular treatment is suitable.

Bring questions that match your goal: whether your symptom pattern needs medical assessment first, what movements to avoid for now, how to coordinate with an existing physiotherapy or medical plan, and what a consultation may involve. If cost is part of your choice, consult the Chinese medicine clinic price list Hong Kong before you enquire. Clear information about your function and priorities lets the practitioner explain options honestly. It also makes it easier to recognise when the appropriate next step is referral or further medical assessment rather than continuing to search for a label online.

Your questions, answered

Frequently asked questions

Does groin pain always mean hip arthritis?

No. Hip-related pain can be felt in the groin, and osteoarthritis is one possible cause, especially when stiffness is also present. But location alone cannot confirm arthritis. Other joint, soft-tissue, abdominal or spinal issues may need consideration, so share the full pattern rather than applying a label yourself. [3]

Why does my outer hip hurt when I lie on my side?

Pressure on the outer hip can make pain from nearby soft tissues or a bursa more noticeable. NHS guidance includes worsening with movement or side-lying in a pattern that may occur with bursitis, but that clue cannot diagnose the condition on its own. [1] Mention the side, onset, activity changes and tenderness at assessment.

Why does a clinician ask about my back when the pain is in my hip?

The lower back and hip region can produce overlapping symptoms. Buttock pain or pain that travels down the leg, particularly with tingling, burning or altered feeling, may lead a clinician to examine the spine as well as the hip. [3] New numbness, weakness or loss of feeling after injury needs prompt medical attention. [1]

Should I stop all activity when my hip hurts?

Not necessarily. For mild symptoms without red flags, NHS advice is to keep moving if you can but avoid overdoing it or putting too much strain on the hip. [1] The safe amount depends on the cause and your function. If walking is worsening, sleep is affected, or you cannot bear weight, seek assessment instead of trying to push through.
