Upper back pain warning signs, upper back pain emergency symptoms and when is upper back pain an emergency
Published Updated
Upper back pain often follows an awkward desk position, a long drive, lifting, sport or a muscle strain around the shoulder blades. Yet location alone cannot tell you that it is harmless: pain in the area from the base of the neck to the lower edge of the rib cage can also be related to the spine, nerves or a problem elsewhere in the body. The key question is what arrives with the pain. Chest symptoms, breathlessness, sudden new weakness, altered bladder or bowel control, major injury, fever or unexplained weight loss are upper back pain warning signs that need medical attention rather than a home remedy. This guide explains upper back pain emergency symptoms and when is upper back pain an emergency, while also helping readers recognise pain that is not an emergency but still deserves an assessment. The short answer
When is upper back pain an emergency? Seek emergency medical help immediately if upper back pain occurs with chest pain, difficulty breathing, fainting, sudden severe illness, new weakness or paralysis, or loss of bladder or bowel control. Pain after a significant fall or collision also needs prompt assessment. Fever, unexplained weight loss, or pain that persists or interferes with daily life should be medically assessed rather than repeatedly self-treated. [1] [2]

Frequently asked questions
Which upper back pain warning signs mean I should seek emergency help?
Seek emergency medical help for upper back pain with chest pain or pressure, trouble breathing, collapse, new weakness or paralysis, or loss of bladder or bowel control. These symptoms can accompany serious heart, lung, nerve or spinal-cord problems and should not be managed with massage, stretching or heat at home. [1] [3]
Is upper back pain after a fall always an emergency?
Not every fall causes a fracture, but upper back pain after a road collision, major fall or forceful blow needs prompt medical assessment. Seek emergency help when there is severe pain, breathing difficulty, confusion, numbness, weakness or another sudden change in function. A lower-impact fall may also need assessment if you have fragile-bone risk factors. [2]
Can I try acupuncture or massage before checking emergency symptoms?
No. Chest or breathing symptoms, neurological changes, a significant injury, fever with severe illness, or other red flags should be assessed medically first. Complementary care may be discussed later for a non-emergency problem after a clinician has considered the cause, but it is not a way to rule out urgent conditions. [1]
When should I arrange care for persistent upper back pain without red flags?
Arrange an assessment if pain is not improving, is worsening, repeatedly returns, or affects work, sleep or ordinary activity. Cleveland Clinic advises professional care if upper back pain does not get better after a week, and earlier review when fever, unexplained weight loss, tingling or numbness occur. [1]
