Shoulder pain causes: Common causes of shoulder pain — What are the common causes of shoulder pain?
Published Updated
Shoulder pain does not always have one obvious cause. It may begin after carrying shopping bags, a gym session or a fall, but it can also build slowly while reaching for a high shelf, turning in bed or working with the shoulders held forward. Common causes of shoulder pain include short-term muscle or soft-tissue irritation, tendon or bursa problems, instability and frozen shoulder. Pain can also be felt around the shoulder when the source is elsewhere. The useful first step is not to label it from a symptom list, but to notice the pattern: when it started, what movement changes it and whether it is limiting sleep, work or ordinary self-care. Sudden severe pain, a changed shoulder shape, a hot swollen joint, ongoing numbness, or pain after an accident needs prompt medical assessment rather than online self-management [1].

The short answer
Common causes of shoulder pain include a muscle or soft-tissue strain, posture-related irritation, tendon or bursa problems, instability, frozen shoulder and injury. The same symptom can occur with more than one condition, so a webpage cannot diagnose the cause. Record onset, triggers, pain location, stiffness, weakness and effects on sleep or daily tasks. Seek urgent assessment for sudden severe pain, deformity, major swelling, inability to move the arm, persistent altered sensation, fever, or pain after trauma [1].

Your questions, answered
Frequently asked questions
What are the common causes of shoulder pain?
Common causes include muscle or soft-tissue strain, posture-related irritation, tendon or bursa problems, instability, frozen shoulder, injury and joint problems. A painful shoulder cannot be reliably diagnosed by one symptom or online list, because several conditions can overlap. [1]
Should I rest completely when my shoulder hurts?
Complete immobilisation is not usually advised for ordinary shoulder pain. NHS recommends staying active and gently moving the shoulder while avoiding activities that make symptoms worse, strenuous self-made exercises and heavy gym equipment. Individual restrictions after injury should come from the treating clinician. [1]
When should I seek urgent help for shoulder pain?
Seek urgent medical help for sudden or very severe pain, inability to move the arm, a changed shoulder shape or severe swelling, persistent pins and needles or loss of feeling, a hot or cold arm, pain after injury, or feverishness and feeling unwell. [1]
Can acupuncture identify or cure the cause of my shoulder pain?
No. Acupuncture does not identify a fracture, dislocation, tear or other biomedical cause, and no treatment can be promised to cure every shoulder problem. It may be discussed as an individual supportive option after appropriate assessment; ongoing medical care should be shared with the practitioner. [3]
