Knee Pain Causes: Inner Knee Pain Causes and Outer Knee Pain Causes

Published Updated

A sore knee after a run, a sharp twinge on the inside when turning, or pain on the outside during stairs can feel very different. Yet knee pain causes overlap more than most people expect. The place that hurts is a useful starting clue, not a self-diagnosis. Think about the first moment it appeared, whether there was a twist or impact, what happens with weight-bearing, and whether the knee is swollen, hot, unstable or hard to straighten. Those details help a clinician decide what needs attention and what can be monitored.

The short answer

Knee pain can follow a strain, overuse, a ligament or meniscus injury, tendon or bursa irritation, osteoarthritis, gout or infection. Inner knee pain causes and outer knee pain causes may involve different nearby structures, but pain location cannot identify the exact condition by itself. Seek urgent medical advice if the knee is badly swollen or changed shape, cannot take weight, locks or gives way, or is red or hot with fever or chills. [1]

Knee Pain Causes, Inner Knee Pain Causes and Outer Knee Pain Causes by Location

The knee is a meeting point for bone, cartilage, ligaments, tendons and soft tissues. A location description helps make a symptom story more precise: front, inner edge, outer edge, back, or deep in the joint. It does not show which structure is responsible. A clinician also needs the onset, activity pattern, visible change, range of motion and relevant health history. If you are tracking symptoms before an appointment, the practical details in our guide to knee pain symptoms can help you describe the pattern clearly.

Knee Pain Causes, Inner Knee Pain Causes and Outer Knee Pain Causes: What Location Can Suggest

Pain at the front of the knee may be noticed with kneeling, prolonged sitting with a bent knee, squatting or stairs. Conditions affecting the kneecap area, a bursa, a tendon or the joint itself can all be considered. Pain at the back may sit with tight surrounding muscles, a tendon or ligament problem, arthritis, or a fluid-filled Baker’s cyst. These are examples of patterns, not a map for naming your own condition. Cleveland Clinic similarly describes location as a clue and advises that assessment is needed to know the cause. [2]

For inner knee pain causes, people often mean discomfort along the medial, or inside, edge. A twist, a direct knock, a meniscus problem, a ligament injury or joint changes may be discussed depending on the story. Outer knee pain causes refer to the lateral edge and may arise after repetitive running, a change in training load, a twist, or other joint and soft-tissue problems. It is tempting to search one side and settle on a label, but that can miss the difference between a gradual ache and a sudden injury. Note whether the pain is pinpointed by touch, appears only after activity, or is linked with swelling or a sense that the knee shifts.

Knee Pain Causes, Inner Knee Pain Causes and Outer Knee Pain Causes During Walking and Stairs

Walking, rising from a chair and using stairs are useful everyday tests because they load the knee in different ways. Pain when stepping down can feel distinct from pain when climbing, while a pain-free level walk does not rule out a problem that appears during bending. Record the activity, the direction of the movement, the duration before pain begins and what happens afterwards. A sudden catch, a pop at the time of injury, a feeling of buckling, or the inability to fully straighten are especially important details to report rather than symptoms to push through.

Avoid assuming that all activity-related pain is osteoarthritis or that every active person has a sports injury. For people aged 45 or over who have activity-related joint pain and little or no morning stiffness, osteoarthritis can be diagnosed clinically in an appropriate assessment; atypical features such as recent trauma, rapid worsening, deformity or a hot swollen joint change the picture. [3] The overview of knee osteoarthritis symptoms may help frame a conversation, but it should not replace an examination.

Knee Pain Causes, Inner Knee Pain Causes and Outer Knee Pain Causes That Need Attention

A careful symptom description is useful, but safety comes first. A knee that has been injured badly, is rapidly changing, or is paired with fever needs medical triage rather than an online comparison. The reason is not that every case is serious; it is that delay can be inappropriate when trauma, instability or infection is possible. The table separates common patterns by the next question to ask, not by a diagnosis.

Function is often the clearest guide to urgency. Compare what the knee could do before the problem with what it can safely do now: stand from a chair, walk a few steps, use stairs, bend and straighten. Do not treat a brief improvement after rest as proof that an injury is minor if the knee later buckles, catches or swells. Likewise, do not dismiss pain because there was no sports accident; repetitive work, kneeling and gradual overload can matter. Write down whether symptoms change over a shift, commute or weekend, and whether one knee or both are affected. Include any sleep disturbance or new limitation in routine tasks. A clinician can put those details alongside an examination and any needed tests.

Knee Pain Causes, Inner Knee Pain Causes and Outer Knee Pain Causes After Injury

After a fall, tackle, awkward landing or sudden change of direction, location is secondary to function. Stop the activity and take note of what happened. Could you continue walking? Did swelling arrive quickly? Is the shape visibly different? Is the knee unstable, stuck or too painful to move? An injured knee can involve a sprain or strain, cartilage or meniscus damage, or a ligament or tendon injury; these possibilities cannot be sorted safely from one symptom alone. The NHS lists inability to move the knee or bear weight, marked swelling or shape change, and locking or giving way among reasons for urgent advice. [1]

Do not repeatedly squat, run or force a locked knee to test whether it will loosen. Protecting yourself from a further fall matters, particularly when the knee feels unreliable. If a significant accident, severe fall, suspected fracture, or complete inability to move the leg is involved, use emergency services appropriate to Hong Kong. For less dramatic but persistent post-injury pain, a medical clinician can decide whether examination, imaging or rehabilitation referral is appropriate. The location may help them focus the examination, while the injury mechanism and loss of function explain why prompt review may be needed.

Knee Pain Causes, Inner Knee Pain Causes and Outer Knee Pain Causes With Swelling or Fever

Swelling has context. A little stiffness after an unusual walk is not the same as a knee that becomes quickly enlarged after a twist, nor the same as a red, hot joint that develops while you feel unwell. Warmth or redness can occur in several conditions, including inflammatory causes, but knee pain with a high temperature, chills or shivering needs urgent assessment because infection must be considered. The NHS specifically advises urgent help for these symptoms. [1]

An episode of very sudden severe pain with warmth and redness also warrants medical evaluation; gout and septic arthritis are among the possible causes listed in official symptom guidance. Do not use massage, intensive exercise, self-needling or a heat pack to try to settle an acutely hot, red knee while waiting for assessment. If swelling is present without those alarm symptoms, record when it appears, whether it follows activity, and whether it is improving. This gives a clinician a more useful timeline than a single pain score and supports a safer choice about next steps.

Knee Pain Causes, Inner Knee Pain Causes and Outer Knee Pain Causes: Assessment and Care

Once urgent problems have been excluded, care should match the working diagnosis, your function and the way symptoms affect daily life. That may involve a medical assessment, physiotherapy or a discussion of complementary care as one part of a wider plan. Bring a short symptom record instead of trying to interpret every ache: side of pain, trigger, timing, swelling, injury history, medication and what you can no longer do comfortably. This is more useful than arriving with a self-diagnosis based on a diagram.

Knee Pain Causes, Inner Knee Pain Causes and Outer Knee Pain Causes in a Traditional Chinese Medicine Assessment

In a Traditional Chinese Medicine consultation, a Registered Chinese Medicine Practitioner may ask about the quality of pain, stiffness, sleep, activity, past injury and changes with cold, damp weather or exertion. They may observe the knee and use palpation as part of a traditional assessment. Terms such as wind-cold-damp bi syndrome, qi stagnation and blood stasis belong to a traditional diagnostic approach. They are not modern anatomical diagnoses and do not identify a torn ligament, infection or another structural condition. Clear disclosure of previous investigations, diagnoses and medicines helps the consultation stay coordinated with conventional care.

Acupuncture can be discussed only after the safety questions have been addressed, and it should not delay assessment of a serious injury or a hot swollen joint. Guidance on acupuncture for osteoarthritis is not uniform across all settings; NICE’s current osteoarthritis guideline says not to offer acupuncture or dry needling to manage osteoarthritis. [3] Our page on acupuncture for knee pain explains why any discussion should be individual, cautious and complementary rather than a promise to treat every type of knee pain.

Knee Pain Causes, Inner Knee Pain Causes and Outer Knee Pain Causes: Heat, Ice and Next Steps

For a minor knee problem without red flags, reduce or modify the movement that clearly aggravates it instead of abandoning all movement indefinitely. A wrapped ice pack may be used for comfort and swelling; NHS guidance suggests up to 20 minutes every two to three hours and advises avoiding long periods of standing. [1] Use heat only as a comfort measure for a stiff, non-acute knee and stop if it worsens symptoms. Keep a cloth between skin and any hot or cold source to avoid skin injury.

If pain is not improving within a few weeks, is disrupting work, sleep or daily mobility, or you are unsure where to start, arrange an appropriate clinical review. For suspected osteoarthritis, NICE describes therapeutic exercise tailored to the person, plus weight management where appropriate, as core management alongside information and support. Your questions, answered

Frequently asked questions

What are common knee pain causes?

Common possibilities include sprains and strains, overuse, tendon problems, ligament or meniscus injuries, osteoarthritis, bursitis, gout and infection. A symptom list is not a diagnosis: the timeline, examination and ability to bear weight determine what should happen next.

Can inner knee pain causes be different from outer knee pain causes?

They can involve different nearby structures, which is why location is worth noting. Inner pain may be associated with issues around the medial side, while outer pain may be associated with lateral structures or repetitive-load patterns. Several problems can still produce similar side pain, so location alone is not enough to identify the cause.

Should I use heat or ice for knee pain?

For a minor problem with no red flags, a wrapped cold pack can be a comfort option when there is pain or swelling. Heat may feel more comfortable for a stiff, non-acute knee. Neither should be used on bare skin or continued if symptoms worsen. A hot red knee or fever needs urgent medical assessment, not home heat treatment.

When should I seek urgent help for knee pain?

Seek urgent medical advice if you cannot move the knee or bear weight, it is badly swollen or visibly changed shape, it locks or gives way, or it is red or hot with fever, chills or shivering. Significant trauma and a suspected broken bone also need emergency assessment.
